Where can I get a military or dependant identification card?

Identification cards can be obtain from the location listed below.  Identification cards are no longer being issued at the Fifth Regiment Armory.  To obtain an identification card, call the number below:

  • Phone number:  410-702-9050

  • Location:  Personnel Service Branch, 13607 Desert Storm Lane, Building 206, Camp Fretterd Military Reservation, Reisterstown, MD 21136

  • Hours of Operation:  Tuesday  – Friday, 8:30a.m to 4:30p.m. (Open during Drill weekends, call to confirm dates)

Appointments are honored and walk-ins are welcome, however, walk-ins are encouraged to call first to ensure the Identification Card System is operational.

What do I need to bring with me to my appointment?

For military personnel, you will need to bring with you:

  • Copy of Enlistment or Oath of Extension, or

  • Promotion Orders, or

  • Retirement Orders 

Military member's appearance must be in accordance with Army Regulation 670-1. (Photo in uniform is not required; however, the standards must be followed)

Male service member’s must be clean shaven or provide a copy of a current permanent profile signed by a military doctor.
 

  • Hats, scarves or other headgear is not authorized.

  • Non-Prescription tinted lens glasses will not be worn in ID photos.

  • Visible clothing must be neutral tone such as gray, black or white & have no discernible design.


Nonparticipating Reserve member’s (IRR, Standby & Retired Reserve awaiting pay at age 60) do not have to be within Service dress & grooming Standards.

(Any orders over 60 days old and not found in the RAPIDS/DEERS  system; will need to be faxed to the project manager and you will have to wait approx 24 – 48 business hours before your information is updated and you can get your ID card.)


For your dependants, you will need to bring with you proof of relationship for dependents (Only original documentation will be accepted):

  • Social Security Cards

  • Marriage license  and certificate with court seal

  • Birth certificate or hospital record (paternity certification via courts if single male)

  • Power of Attorney (if sponsor is not available to sign 1172)

  • Original signed 1172 by the sponsor.


What is the procedure for a replacement identification card?

Bring with you:

  • Memorandum from unit, signed by the commander (Lost or stolen Common Access Card or Green ID card)

  • Other form of ID (drivers license, State or government ID card)

  • Police report of thief / notice from MP statement of confiscation.

  • DA Form 4856 (developmental Counseling Form) (Lost/Stolen CAC ID)


If you would like to obtain your dependent identification card at Fort Meade, please read the information below:

You  must enter Fort Meade through the gate located at Route 175 and Reese Road.  Bring with you the registration certificate and insurance card for the vehicle you are in and a picture identification, i.e. driver's license.  You will be given a one day pass which will allow you to obtain an identification card.  The Identification Card Section hours are:  7:30a.m. to 3:00p.m
